Release of Liability *

I have read the release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.

By signing this Waiver, I acknowledge that I am in good health and in proper physical and mental condition to accept the services, goods, and arrangement for services such as emergency shelter or housing, and I assume all risk, and take full responsibility, waiving any claims of personal injury, disability, sickness, loss or damage to person or property inherent in, and/or in any way connected with participating in these activities by Women That Give.

The participant releases the organization, Women That Give and its representatives from any claims or liabilities arising from participation in the activity/event/ or for arrangement for services/ or for items received.

Indemnification *

I waive my right to sue this organization Women That Give, WTG or their staff, volunteers, the owner, board, or the leaseholder of the building for injuries or damages I may sustain.

The participant agrees to indemnify and hold the organization harmless from any claims or for death, personal injury or property damage of any kind brought against the organization as a result of their actions or negligence.

Assumption of Risk *

I acknowledge the risk of my participation in this activity or arrangement of services I receive.

The participant understands and acknowledges the inherent risks associated with the activity/event/arrangement. The participant releases the organization and its representatives from any claims or liabilities arising from participation in the activity/event/arrangement for services/items received.
